Sugarman Health & Wellbeing Nurse Assessor

* Ensure that professional practice standards and "best practice" are maintained in all areas of work with guidance from the clinical lead
* To deliver productivity and quality standards agreed between yourself and the management team and to respond positively to feedback
* To undertake recorded clinical assessments where required
* Management and oversight of all clinical care packages including clinical reviews, carer management and clinical competencies within scope of practice
* To work unsupervised and use own initiative however know limitations and ask for support where necessary
* To analyse and interpret clinical information and medical evidence and provide a report in a professional and concise manner

* Work well as part of a multidisciplinary team and build positive working relationships, including other nurse assessors, carers and co workers within the office environment
* Complete training, competency assessments and supervision sessions for the teams allocated to the clinical services
* Apply professional skills and manage own professional competence and accountability, in accordance with the NMC Code of Conduct
* Keep up to date in accordance with National and local policies.
* Report to Clinical lead regarding the above and what training or updates you may require to ensure you stay up to date and re-validate in accordance with NMC regulations/ Standards.
* In addition to the responsibilities listed above, the job holder may be required to perform other tasks as delegated.


* Provide continued clinical input and support for staff in the daily delivery and support for individuals in their own home, ensuring that each person supported has their individual clinical care plans implemented and evaluated as required, in order to accommodate to their changing needs. Update care plans & risk assessments as necessary as individuals care needs change.

* Effectively delegate and supervise care staff in order to ensure all identified clinical and care needs are met.

* Follow all processes, policies and procedures of the company at all times

* Have a clear understanding of personal accountability, undertake clinical procedures and training with individuals in accordance with care plans and sign off required competencies in a timely safe manner.

* Recognise and respond appropriately to urgent and emergency situations, escalate as necessary and document findings/ outcomes.

* Providing verbal and written reports/review on service issues and individual's progress to the Registered Manager and Clinical Lead as appropriate.

* Complete training, competency assessments and supervision sessions for the teams allocated to the clinical services.

* Report real time incidents, accidents or complaints to the Manager in charge, in line with company policy and process and make necessary regulatory notifications in consultation with management.

* Ensure care plans are made individualised and use guidance from local/national guidelines for clinical needs of the individual.

* Write MAR sheets in accordance with National guidance and in conjunction with individuals multi disciplinary team for individual client, to ensure MARs are up to date and correct. Report and document incidents found on monthly audits to Registered Manager and Clinical Lead. Also address issues raised as per company policy.

* Record, audit and complete any appropriate documentation and records/systems in order to ensure that individual's records are accurate and informative.

* Communicate and coordinate with other Health and Social Care Professionals and Medical Practitioners, to ensure a comprehensive and cohesive clinical approach in all planned actions and interventions for the health needs of the individuals you support.
