Overview

This post is based at A University Hospital based within the Munster Region of Ireland.

The region has a population of approximately 178,000 and the hospital is question also provides certain health services for the population amounting to some 55,000.
The University Hospital provides Acute Medical and Surgical, Maternity, Paediatric, Geriatric, ITU and Out Patient facilities. There are eight operating theatres for General, Vascular, Urology, Dental, Gynaecology, Trauma, Orthopaedic, Day Care, Ophthalmology and Pain procedures.

A dedicated Emergency CEPOD Theatre is open 24 hours a day.

The Hospital has a busy Accident & Emergency Department. A purpose-built Day Procedures Unit with 3 theatres. The Maternity Hospital, which houses its own Operating Theatre on Labour Ward, undertakes some 2,400 deliveries per annum.

The Anaesthetic Department provides a 24 hour Epidural Service. The Anaesthetic Department provides cover for radiological procedures requiring general anaesthesia or sedation, for emergencies requiring a CT scan and for cardio-versions. New CT and MRI scanners were recently installed in a purpose built suite within Radiology. A recent development for the hospital and the Department was the introduction of day case ophthalmic surgery.

There is a newly built and superbly equipped Post Graduate Centre housing a modern, comprehensive Library and Seminar Rooms with state of the art audio visual and computing facilities. It boasts a clinical skills laboratory and a computer-training centre.

Rehabilitation facilities are available on the hospital site and include occupational therapy and Out Patient Pain Medicine programs.

Specific Duties

To meet the changing demands of patient care and service delivery patterns it is possible that some flexibility will be introduced into all Consultant work programmes. The theatre lists are provisional and may be altered after discussion if necessary. An element of flexibility in the job plan is crucial for the smooth running of the Service to meet the contractual commitments of the University Hospital and changing service demands on a weekly basis.

A Job plan is outlined is the ‘Job Plan’ of this job description indicating clinical sessions. This post will attract 10 PA’s of which 8.5 PA’s will be for direct patient care. Further clinical sessions may be undertaken by mutual consent and with suitable recompense.
This job plan may be amended in consultation with the post holder as hospital developments evolve, and as part of the appraisal process.
Intensive Care /HDU

The ITU / HDU complex is built on a nucleus plan and is self-contained, with a comprehensive range of facilities – including Doctors’ and Relatives’ overnight stay rooms within the complex.

There are 4 ITU and 4 HDU beds, although this changes with the dependency of the patient. The two isolation beds tend to be used for “quarantine” purposes.
Working relationships within the multi-disciplinary team are excellent with patient care being shared with other specialties. The Outreach Service is well established with very active and enthusiastic team of senior ITU staff.

The department contributes to the National Audit project and is a member of the HSE Network, which includes General Hospital and University hospitals within the region. A separate Transfer Group exists. Haemofiltration is carried out on site, with support from visiting nephrologists.

The Unit is recognised for training of HSE structured course nurses.

The unit underwent an extensive refit with the installation of a Draeger gantry system to all 8 beds and new monitoring.
This appointment will enable the department to work towards covering all weekly sessions by Consultants with a Critical Care interest. The successful applicant will work with his or her colleagues to ensure continuity of clinical care within the Unit.

Obstetric Facilities

Obstetric care is delivered in the Obstetric Block on the hospital site. The Department at present provides Consultant cover for 10 sessions a week, 5 of these are elective operating lists. NCHD’s take part in obstetric care at Registrar level.

The labour suite includes delivery beds, a dedicated operating theatre with full facilities for the administration of general and regional anaesthesia. A purpose built high dependency room serves mothers for complicated deliveries.

Obstetric services are provided by the Directorate of Obstetrics, which includes 4 Consultants.

Pain Services

The Chronic Pain Medicine service integrated within the Anaesthesia Service is currently provided by two Consultants within the Department, assisted by a Chronic Pain Nurse Practitioner, a Pain Sister and Staff Nurse. An Acute Pain Service is provided by two Consultant Anaesthetists and an Acute Pain Sister.

Teaching Commitments

• regular commitments to ward based teaching of undergraduates.
• participation in the lecture programmes for 2nd and 4th & 5th year medical students.
• teaching of other groups as required including other specialty juniors and specialist nurses.

Job Plan

A formal job plan will be agreed between the appointee, the Operational Medical Director and Lead Anaesthesia Consultant, on behalf of the Trust’s Medical Director, based on the provisional timetable. This will be signed by the Chief Executive and will be effective from the commencement date of the appointment.

It will be reviewed at 3 months & thereafter annually or at any time, but no less than 3 months after a previous review, as requested by the appointee, Operational Medical Director or Clinical Lead & adjusted accordingly to the agreement of both parties.

Direct Clinical Care:

6 DCC = 7.50 PA
SPA = 1.5 PA
1:14 on call = 1.0 PA
TOTAL = 10 PA

On-call Duties:

1.0 PAs are included each week for on-call duties. This was established from Consultant-on-call diaries and will
be continuously reviewed. Consultant Anaesthetists currently work 1:14 on-call rota with prospective cover. After hours duties include Intensive Care and Maternity cover.

The department is ultimately committed to separating the consultant-on-call for ICU and Maternity.

Standard Duties and responsibilities

a) To participate in development of and undertake all duties and functions pertinent to the Consultant’s area of competence, as set out within the Clinical Directorate Service Plan and in line with policies as specified by the Employer.

b) To ensure that duties and functions are undertaken in a manner that minimises delays for patients and possible disruption of services.

c) To work within the framework of the hospital / agency’s service plan and/or levels of service (volume, types etc.) as determined by the Employer. Service planning for individual clinical services will be progressed through the Clinical Directorate structure or other arrangements as apply.

d) To co-operate with the expeditious implementation of the Disciplinary Procedure

e) To formally review the execution of the Clinical Directorate Service Plan with the Clinical Director / Employer periodically. The Clinical Directorate Service Plan shall be reviewed periodically at the request of the Consultant or Clinical Director / Employer. The Consultant may initially seek internal review of the determinations of the Clinical Director regarding the Service Plan.

f) To participate in the development and operation of the Clinical Directorate structure and in such management or representative structures as are in place or being developed. The Consultant shall receive training and support to enable him/her to participate fully in such structures.

g) To provide, as appropriate, consultation in the Consultant’s area of designated expertise in respect of patients of other Consultants at their request.

h) To ensure in consultation with the Clinical Director that appropriate medical cover is available at all times having due regard to the implementation of the European Working Time Directive as it relates to doctors in training.

i) To supervise and be responsible for diagnosis, treatment and care provided by non-Consultant Hospital Doctors (NCHDs) treating patients under the Consultant’s care.

j) To participate as a right and obligation in selection processes for non-Consultant Hospital Doctors and other staff as appropriate. The Employer will provide training as required. The Employer shall ensure that a Consultant representative of the relevant specialty / sub-specialty is involved in the selection process.

k) To participate in clinical audit and proactive risk management and facilitate production of all data/information required for same in accordance with regulatory, statutory and corporate policies and procedures.

l) To participate in and facilitate production of all data/information required to validate delivery of duties and functions and inform planning and management of service delivery

Essential Requirements

This post is open to doctors who hold Registration as a Specialist in the Specialist Division of the Registrar of Medical Practitioners maintained by the Medical Council in Ireland in the specialty of Anaesthesia

Office Accommodation

The post holder will undertake administrative work associated with their clinical and other professional duties. Adequate time and facilities for clinical administration, including appropriate office space, secretarial support and access to a personal computer, software & internet access, will be available.

Management Responsibility

All Consultants are required to attend the monthly Directorate Meetings. Post holders will be expected to share in administrative duties allocated by mutual agreement within the Directorate. As part of the Consultant Team, the consultant will offer mentoring support and lead the existing clinical team through; joint clinics, one-to-one support, individual case management and complex cases, identifying development needs and suggesting CPD (Continuous Professional Development) opportunities and responding to clinical enquiries via e-mail and telephone.
