Re-enablement Support Worker

Medacs Healthcare is currently recruiting for experienced, compassionate and committed individuals to join our ever expanding teams in and around the Trafford area.

Candidates must be trained to NVQ /QCF level 2 in Health & Social Care as a minimum and have a flexible and enabling approach.

The purpose of the role is to provide early intervention, following discharge from hospital/respite care, also, providing preventative services with the goal of enabling people to remain in their own home and to continue to live an independent socially inclusive life.

The Role:

* To provide pro active, emotional and practical support to clients in their own home.
* Assisting clients to set and achieve their own personal goals i.e. improving mobility and ensuring social inclusion.
* To work closely with other healthcare professionals i.e. physiotherapists, GP's etc.
* To support clients to achieve independence and fulfil their potential.
* Supporting positive risk taking.
* To assess need and review progress on outcomes/goals on a daily basis.
* To ensure accurate and detailed documentation is communicated to the appropriate agencies.
* To contribute and provide written and verbal reports at multi disciplinary team meetings.
* To give the service user increased control over their own life and instil hope for the future.

Required Skills:

* The desire to assist clients to achieve their full potential.Trained to NVQ/QCF level 2 in Health & Social Care.
* Excellent communication skills, both written and verbal.
* Experience of delivering "hands on" care to vulnerable adults.
* To have a flexible approach to work and a "can do" attitude.
* To understand the needs of service user's and their families.
* A full UK driving licence and unrestricted access to a vehicle.

Desired Skills:

* Experience of writing Support Plans, Risk Assessments etc.

* Experience of working in partnership with other healthcare professionals.

* Willingness to work towards NVQ/QCF level 3 in Health & Social Care.

Benefits:

* Block Shift payments.

* Enhanced pay rates: £9.00 per hour Monday to Friday, £9.50 per hour Weekends, £13.50 per hour Bank Holidays.

* Full training will be provided including The Care Certificate.

* Paid DBS, refunded after 100 hours of work.

* Block shift payments - 07.00 to 14.00 and 16.00 to 22.00.

* A mixture of shift patterns, between the hours of 07.00 to 22.00 plus alternate weekends on a weekly rota basis.
