Purpose of the role:

To establish the exact care requirements of the Service User by observations and assessments and to put in place adequate support plans to ensure the Service User can remain at home safely and to avoid any further hospital admissions.

This is a 22 week winter pressures contact to prevent hospital admissions. Although this is a 22 week contract, further opportunities will be available.

Key skills of candidates:

Minimum of NVQ/QCF 2 in Health & Social Care.

Highly skilled and experienced care worker.

Person centred thinking.

Outcome focused.

Ability to communicate with all stakeholders i.e GP's, Social Workers, O T's, other care staff, family members etc.

Full UK driving licence with unrestricted access to a vehicle.

Job Description: - Field Based Role

Service User is given a package of three nights care/observation.

Support Worker to observe the service user throughout the evening as required, to record findings and to report back to the coordinator on a nightly basis.

To provide personal care or any assistance in re-positioning the service user.

To ensure the safety and security of the service throughout the night.

To provide fluids and medication as required.

Any other ad-hoc duties.


Block payments 22.00 pm - 08.00 am

£9.00 per hour - £9.50 per hour and £13.50 Bank Holidays.

Full training up to Senior Carer level including Risk Assessment and Care Plan training.

Enhanced DBS paid for by employer (re-imbursed after 100 hours).

Company pension scheme.

25 paid holidays per year.
