Reporting to the Clinical Business Manager the postholder will lead and manage the Capacity Management team (bed managers) and the Integrated Discharge Team based at the Hospital site including co-ordination of the discharge of patients with complex needs within a single assessment process and timely transfer across the 7 day week. The post holder will ensure the principles of single point, early and effective discharge ensuring the requirements of the Health and social care Act, 2014 are met as well as compliance with relevant Trust policies.
The post holder will foster a culture of good inter-disciplinary and organisational working, constantly demonstrating high standards of professional behaviour, acting as role model for the wider team and acting as a positive ambassador for the Hospital, ensuring the expected standards of behaviour are integrated within the Teams at all times. Ensure office facilities demonstrate high standards of business presentation at all times and all team members are compliant with the Trust's Standards of Work Ware' policy.
Provide professional leadership to members of the team, nurses, AHPs and ensure a process is in place to maintain the necessary professional standards for Social Workers.
Responsible for risk management, clinical governance, information governance, training and compliance.
Supported by the teams they lead, provide expert advice and guidance to wards alongside supporting effective patient flow, capacity and demand management, together with on-going monitoring and discharge of patients across pathways applying the principles of 'Home First', ensuring reduction of dependency on bed based services as a solution.
Provide regular reports of a high standard, within an agreed framework that demonstrate capacity/ demand, actual or predicted service deficit for all services, together with quality standards which demonstrate timely transfer making recommendations for improvements in pathways/ systems and demonstrating value for money.
Responsible for the development, updates and compliance of operational policies, plans and standard operating procedures, including but not exclusively 'SitRep' validation and reporting and application of 'Reimbursement' and escalation.
To support and contribute to the on-going service review and development.
The Post holder will be the expert for all complex discharge matters within the Trust and act as a conduit for all guidance and information. The post holder will work autonomously towards defined results.
