An exciting opportunity has just arisen for a Band 5/6 Community Psychiatric Nurse (CPN) to work in a Community Mental Health team in Birmingham.

It would be on a full time basis - 37.5 hours each week - starting ASAP for an initial contract of 3 months.

Brief duties:

* Care coordination for patient on CPA
* ICR completeness
* Care planning
* Assessment and duty roles within CMHT
* Visiting patients at home environment
* Liaising with other teams and inter agency working
