Band 7 Locum Neuronavigator - Physiotherapist

National Locums are urgently looking to recruit a Band 7 Locum Physiotherapist (case manager for complex neurorehabilitation clients in community).

The ideal candidate will be required to cover a complex neurorehabilitation role so experience within experience in neuro rehabilitation, ideally a combination of specialist inpatient neuro-rehabilitation units, community neuro-rehabilitation teams, acute neurology wards. MDT experience, leadership this area is a must.

Job Description:

Our NHS Client in Central London require a Band 7 Physiotherapist to join their Rehabilitation and Therapies Community Team.
Job Summary

• Supporting, informing and empowering patients and their families to navigate the complex neuro-rehabilitation pathway whilst ensuring the most effective use of resources

• Co-ordination of admissions of patients with complex neurological disabilities requiring neuro rehabilitation to Specialist Neuro-rehabilitation Units and acute hospitals with efficient use of local services ensuring that neuro-rehabilitation offered is clinically appropriate
• Robust discharge planning from acute hospitals and specialist inpatient rehabilitation units that meets the estimated date of discharge and ensures that a holistic consideration is made of clients’ bio/psycho/social needs
• Timely onward planning for community rehabilitation making efficient and creative use of available resources
• Audit and data analysis to assist forward planning of the service
• To provide education, training and leadership to health and social services working with this group of service users, in order to achieve more integrated cross-organisational case management

Applicants must have:

- Min 12 months relevant NHS experience
- HCPC registration
