Clinical Pharmacist position in Bristol, Monday to Friday, 37.5hr week, up to £53,219 per annum.

As a high-achieving and well-organised practice, we embrace a Multi-Disciplinary Team approach to care provision. We are looking to establish a new pharmacist hub covering Bristol and South Gloucester, replicating what we already have in North Somerset. You will be part of a team providing expertise for the medicines management processes for the group

As a Clinical Pharmacist at a high-achieving and well organised practice you will:
Work up to 37.5 hours per week, Monday to Friday and be supported with the development of your Clinical Career.

Key Duties
The post-holder will develop good working relationships with:

* Patients
* GP, nurses and other practice staff

Clinical Responsibilities:

* To undertake a variety of duties, including working across the practices in their area as an expert for the medicines management processes.
* Working across sites to optimise processes to achieve highly effective, safe and patient centred prescribing.
* To work closely with the Senior Clinical Pharmacist in the delivery of our business priorities with a practice-based focus.
* Working towards or as an independent prescriber, to undertake patient consultations with a focus on groups of patients with potential medication issues, whilst prescribing in accordance with the practice prescribing formulary (or generically) whenever this is clinically appropriate.
* Recording clear and contemporaneous consultation notes to agreed standards.
* Recording appropriate Read Codes to maximise the practice QOF performance, and participate in QOF recall reviews.
* Collecting data for audit purposes.
* Compiling and issuing computer-generated acute and repeat prescriptions (avoiding hand-written prescriptions whenever possible).
* Other local practices clinical pharmacist
* Community pharmacists and support staff
* Hospital staff with responsibilities for prescribing and medicines optimisation
* Members of the CCGs medicines management team.

Key Skills, Qualifications and Client Requirements
Qualifications
Essential

* Postgraduate Diploma or Masters Degree in Clinical Pharmacy (MPharm), and registration with the General Pharmaceutical Council (GPC)
* Member of the Royal Pharmaceutical Society (RPS)
* Evidence of continued learning/personal development in relevant field

Desirable

* Hold, or be working toward, an Independent Prescribing Qualification

Experience
Essential

* Previous evidence of Pharmacy roles in either the NHS or commercial environment
* Demonstrates general pharmaceutical skills and knowledge in core areas
* Experience of developing and leading implementation of service delivery policies and procedures
* Evidence of up to date and relevant knowledge of Primary Care provision
* Sound clinical knowledge and understanding of the principles of evidenced-based healthcare

Desirable

* Has awareness of the breadth of common acute and long-term conditions that are likely to be seen in a General Practice setting
* An appreciation of the nature of GPs and General Practices
