Type of role: Telephone Triage Nurse - Clinical Care Coordinator

Contract: Full-time, Permanent

Shift patterns: 37.5 hour week over 4 shifts

Location: Naas, Co. Kildare - WFH / hybrid model being trialled.

I am delighted to assist my client, a leading Health Insurance Group, in recruiting for Clinical Care Coodinator's to join their Health & Wellbeing team in Kildare. My client has developed a Clinical Call Centre (CCC) to facilitate triage and navigation of client/patients throughout the Health & Wellbeing services.

About the Role:

The Care Coordinator Nurse facilitates callers' by providing access to the appropriate level of care using telephonic interaction as the mode of communication, by assessing needs, offering a clinical option, i.e. appropriate referral pathway to healthcare providers and services. The Care Coordinator Nurse must demonstrate the ability to use the nursing process and independent decision making in supporting the non-clinical teams.

Eligibility Criteria:

* Must be registered with the NMBI.
*
* Experience of working within a Call/Contact centre desirable.
* Demonstrates ongoing evidence of Continuous Professional Development.
* Demonstrates an active participation in education and mandatory training.
* Maintains current knowledge base, including triage skills, and appropriate teaching principles for all age groups using the CCC.
