What are the key responsibilities?

●The post holder will provide clinical care alongside the practice teams consisting of clinical lead, general practitioners, practice nurses and Healthcare assistants.
●Patient contact will be via patient facing clinics, triage, on-call duties and home visits
●Duties include, but not limited to: home visiting service, home visits, post-discharge reviews and "minor illness" service
