Job Code : AA07070809 Our client is looking for a Qualified Socioal Worker to work in their Community Mental Health team in South West London. The Successful candidate will have a minimum of 1 years post qualifying experience and be up to date with current legislation.

The role will include the following:

? Working with adults with severe and enduring mental illness
? Packages of care formulated via the Core Programme Approach with an allocated care
coordinar
? Emphasis on community assessments (particular emphasis on care assessments)
? Developing links with community based statutory and voluntary agencies
? Attending Mental Health Review Tribunals
? Arranging CPA reviews and taking part in the teams duty system
? Working within a multi disciplinary team, incorporating social work, community nursing,
psychology and occupational therapy

* A Social Work Qualification
* GSCC Registration
* Enhanced CRB
* Details of referees going back 5 years
* knowledge and experience relevant to Mental Health Social Work
