Medeeca are currently seeking a Liaison Nurse to join one of our private Clients modern, well equipped hospitals based in London.
The ideal candidate will be working in the Neuro rehabilitation department of the hospital.
The position requires the successful candidate to liaise with rehabilitation reservations department in order to gain pre-admission information on the patient to identify possible difficulties that may occur in transfer of care and to liaise with the interdisciplinary team, consultant, patient and family in order to formulate the discharge plan.
The successful applicant will also be expected to assist in carrying out the discharge plan by making appropriate referrals and following them up to ensure safe discharge of patients.
The successful candidate will have First Nurse Level 1 or be an Allied Health Professional with specialist experience in rehabilitation alongwith knowledge of current legislation. A good standard of written and oral English is essential.
